Who controls your information?
The assessor or organisation that sent you the questionnaire is the data controller for the information you provide. They decide why the information is collected, how it will be used, who it will be shared with, and how long it will be kept.
Assessment Questionnaires provides the platform and processes your responses on behalf of that assessor or organisation. This means we normally act as their data processor for your questionnaire responses and related assessment records.
What information may be collected?
The questionnaire may ask for information relevant to an assessment, such as education, learning, development, health-related information, family learning history, school support, strengths, difficulties, and views from home or school. Only provide information that is relevant to the assessment.
How will your responses be used?
Your responses will be made available to the assessor or organisation that requested the questionnaire. They may use the information to support assessment planning, professional judgement, report writing, and communication with relevant parties involved in the assessment.
Who can see your responses?
Access is limited to authorised users of the organisation that created the assessment and to platform administrators where necessary to provide support, maintain security, or operate the service. The assessor or organisation is responsible for deciding whether assessment reports or questionnaire information are shared with others.
How long is the information kept?
The assessor or organisation controls retention of assessment records. If you have questions about retention, access, correction, deletion, or sharing, contact the organisation that sent you the questionnaire.
Your rights
You may have rights to access, correct, erase, restrict, or object to processing of your personal data. Because the assessor or organisation is the controller, requests about your questionnaire responses should usually be directed to them first.
